网上调查问卷的设计

下面是澳大利亚的一个网上调查问卷，笔者认为设计的比较好，可以作为问卷设计的参考。

Hello all
Last year the Strategic Planning Committee developed a survey of our
members. Due to technical and then timing issues that survey was never
sent to you.

As we are still interested in finding the answers to the survey
questions, we are now asking you to complete the survey on line (about
10 minutes). http://www.qedml.com.au/index.php?SURVEY=unimelb2

Unfortunately, I no longer seem to have access to that part of the
software that will enable me to change the words on the survey.

So, please ignore the fact that it is called the 2005 survey!

Please complete the survey by Friday 17 March.
